These devices can be installed using existing plumbing or with an addition, and the amount of work that is put into the installation will be reflected in its efficiency. The simpler solution will use your cold water pipes to return the hot water back to the heater, while the more complicated system has its own loop where the water circulates. The idea is that the hot water will be right there when you turn on the faucet, but in a system that uses the cold water pipe for circulation, you will have issues with the cold water taking a minute to get cold instead.
